Safety Summit & Open House

I certify and acknowledge that:

1. I am at least eighteen (18) years of age and am voluntarily participating in this site visit/demo/skills challenge with knowledge of the risks involved.
2. I will conduct myself in a safe and appropriate manner, consistent with all laws, regulations, and Ashley Sling’s safety and other requirements.
3. I (for myself, my relatives, heirs, executors and all other persons) hereby release, hold harmless and indemnify Ashley Sling, The Crosby Group LLC, its affiliates, subsidiaries, officers, directors, sponsors and any volunteer or support personnel from and against any and all rights and claims for personal injury, death or property loss or damage caused wholly or in part by any act or omission of Ashley Sling.
4. I do hereby fully and freely consent to the use by Ashley Sling, Crosby and the distributor of my name and photograph for advertising, display, or other use. I do hereby release and hold harmless Ashley Sling and Crosby and its agents and assigns from any liability arising out of said participation in a Ashley Sling or Crosby publication, advertisement, and/or promotion, or other use.

I fully understand that the beneficiaries of this release are and will be relying on my consent as evidenced by my signature.

I voluntarily waive any and all claims, both present and future, which I, or anyone on my behalf, may have or claim to have against Ashley Sling / Crosby, arising from or related to my participation in this event, including but not limited to, claims for negligence, property damage, personal injury and/or wrongful death.
